Transformative Deep Plane Results: The Benefits of the 5 Layer Facelift
For patients who want real rejuvenation without sacrificing facial identity, the 5 Layer Facelift is a game-changer. Unlike the extensive incisions and artificial appearance of traditional facelift techniques that tightly pulled lax facial skin backwards. Dr. Shah’s 5 Layer Facelift is a highly customized integration of deep plane and SMAS facelift surgery techniques that offers the following benefits:
- A meticulous approach to facelift incision size and placement for superior concealment.
- Rejuvenation of all five anatomical layers—skin, fat, muscle, fascia, and underlying ligaments.
- Vertical lifting techniques that deliver better jawline definition and profile contouring.
- Deep plane techniques for targeted tightening and repositioning that reverses facial aging starting from the upper cheeks and moving downward through where the area under the chin intersects with the top of the neck.
- Less trauma to the surface layers of facial skin allows for faster healing and the potential for a quicker return to work and social activities.
- Long-lasting results that restore a youthful appearance with an outcome that is aesthetically superior—you’ll look great, but not like you had facelift surgery.
Why Choose Dr. Shah For 5 Layer Facelift?
A distinctive feature of Dr. Shah’s facelift approach is his tailored incision strategy, adapting to the individual anatomy of each patient. He skillfully hides incisions in carefully chosen locations, such as within the hairline or along natural skin folds, to reduce scar visibility and maintain the patient’s natural look. Whether he’s conducting a conventional facelift or a neck lift, Dr. Shah is meticulous about positioning scars inconspicuously. This allows his patients to experience the revitalizing benefits of the surgery without obvious signs of the procedure.
For patients worried about noticeable scars, Dr. Shah provides neck lifts featuring incisions that are either hidden or minimally visible, tailored to their specific requirements and desires. Employing sophisticated surgical methods and a keen eye for detail, he delivers impressive outcomes with little sign of the surgical procedure. This method is especially favored by executives and professionals in New York City, where maintaining a polished appearance is often crucial for professional and social success.

She looked me in the eyes..and she told me, “I want to look like her again.”
A lot of patients see me for their revision work. One patient in particular, who happens to be a
former Playboy centerfold model, has instilled her trust in me to bring back the version of herself
that was at one point thought to be lost to time.
Going beyond the standard “before and after” documentation, I’d like to go a bit more in depth about this surgical process and provide you with the realities of performing such a unique procedure. The structure and path were all there, so this felt to be a fun challenge of turning her back into herself.
On top of doing a restoration of her features, I also had to factor in that my client wanted these results completed within a one month time frame. She wanted to get back to work as soon as possible, as she also works in the aesthetics industry.
She’s been in the industry for years. In the past, she’s gotten a face lift, lip lift, eye lift, and filler. Unfortunately, a lot of this work did not turn out the way that she had envisioned. Going into this process, I recognized that there were a number of things right off the bat that I personally did not think fully maximized her beauty to the best potential.
Step by step, what procedures were done to recreate her iconic look?
To restore these stunning features, what we had to do was remove excess skin around the lip, eyes, and brows. I always advocate for a brow lift to coincide with an upper blepharoplasty, as eyes often tend to fail within the corner, where the gland is out.
She had gotten a blepharoplasty done in the past, and I could tell that this was unfortunately the case for her as well. By balancing out her brow, I took off the heaviness without having her look over-arched.
Her original lip lift from a previous surgeon was not effective, which also left a scar that I had to reposition to hide.
The problem with most lift lips is that the scars are in a bad location. They go straight across, rather than hidden inside the nose. When we did her lip lift, I had to shorten some distance to make it look exactly like she did in her past photo.
I did my patented 5-layer neck lift on her, where I improved the deep contouring of the neck. This targeted the aging in all five structural layers of her neck, lifting her physically and improving her hyoid along the way. We were able to create a sharper angle along her jawline, which is the most iconic feature that landed her jobs in the past.
I also revised her face by doing a deep plane facelift. A lot of surgeons may say that they do a deep plane lift, but they don’t actually do so. You can see by the way there is tension in her scars that this was unfortunately true for my patient as well.
It was clear that what we needed to do was balance two important shadows across her face; the OG Shadow and the Triscural Shadow. This improved her Tragus contour, giving the face a proportional, less flat look.
Recovery
The recovery process was really important for this patient. As I mentioned before, she needed these results ready within one month. Most people talk about having wraps, drains, or maybe even using tissue glue. We decided to go above and beyond that. We prepped her with supplements and a diet plan, to make the recovery as seamless as possible. In addition, we used a special continuous cooling device, alongside applying exosomes topically around the incisions to help her scars heal faster. This is her at day one:
The first 24 hours are crucial for a speedy recovery. She reported little-to-no pain, starting to maintain a high protein diet along the way.
She followed these guidelines that we set in place. You can see already, she looks great and is already feeling much more confident.

FAQs: 5 Layer Facelift Surgery in NYC
What is Dr. Shah's preference: filler or cartilage for nose augmentation?
A consultation is necessary to determine if the procedure is right for you based on your skin structure, overall facial anatomy, aesthetic goals, and medical history. Ideal candidates for Dr. Shah’s 5 Layer Facelift approach include:
- Patients with moderate to severe signs of facial aging such as:
- Sagging skin in the lower face
- Deep nasolabial folds
- Jowls or loss of jawline definition
- Skin laxity in upper neck skin or prominent neck bands
- Adults in good general health who are non-smokers or willing to temporarily abstain from all forms of tobacco prior to surgery and during recovery.
- Individuals whose aesthetic goals are beyond the results potential of non-surgical treatments such as fillers, skin tightening devices or PDO Threads.
- Patients with realistic expectations of what a facelift addresses to ensure a satisfying outcome.
Individuals with prominent aging throughout the neck may want to consider expanding their facelift surgical plan to include rejuvenation of the 5 layers of the neck, for a combined approach sometimes referred to as an extended facelift. This combined approach avoids a noticeable contrast between the face and neck.

How is the deep plane or 5 layer facelift performed?
The consultation appointment is where you’ll discuss Dr. Shah’s surgical approach, along with determining the location of your surgery, if it will be performed as an outpatient procedure, anesthesia and additional treatment options and the approximate of time needed to complete your procedure, in the meantime here is a general overview:
- General anesthesia or twilight sedation
- Meticulous incisions are made within the hairline and around the ears that with proper healing will be hidden and barely visible to even you, and only because you know where they are.
- Advanced surgical techniques are used to address the five key layers of the face, skin, fat, muscle, SMAS (superficial musculoaponeurotic system) ligaments and fascia, ensuring a tightening and repositioning delivers youthful definition with lasting enhancements.
- Overlying skin is then addressed, Dr. Shah’s advanced surgical skills and expertise in reshaping the overlying skin and trimming away the right amount of excess tissue is a key advantage at this step, ensuring incision suturing and healing without creating tension or pulling that will look unnatural.
- Bandages and compressive wrapping or specialized facelift support is applied, you will then be monitored for a few hours in a post-operative recovery area before being released and moving on to either in-patient or out-patient overnight care as pre-arranged.
What should I expect during facelift recovery?
- Days 1–7:
Expect swelling, bruising, and mild discomfort. These symptoms are normal and generally manageable with prescribed medication and rest.
- Week 2–3:
Many patients feel comfortable enough to return to light work, social events, or Zoom calls—especially with makeup or strategic hairstyling.
- Weeks 4 and Beyond:
Most residual swelling resolves. Results continue to improve over the next few months as deeper tissues settle and incisions continue to heal and fade.

- Infection – Rare, but possible; typically treated with antibiotics.
- Nerve Injury – Temporary numbness or weakness may occur; permanent damage is extremely rare.
- Poor Wound Healing or Scarring – More common in smokers or those with certain medical conditions.
- Prolonged Asymmetry – Mild asymmetry or contour irregularities do not resolve during the first month of healing can happen but are usually minor and correctable.
When to Contact Your Surgeon Immediately:
- Excessive swelling or bruising on one side of the face
- Severe pain that isn’t relieved by medication
- Sudden increase in swelling or tightness
- Signs of infection – fever, chills, redness, warmth, or discharge from incision sites
